• Unexpired NYS ID (Original copy)

• SS Card (Original copy)

• PCA/ HHA Certificate

• 2 Reference Letters (1 Personal and 1 Professional)

• Physical Stamped (within current year)

• Flu Vaccine (recommended)

• Laboratory Work **(must attach printed lab reports) **

• Drug Screening Lab (within current year)

• Rubella Lab (not immune 🡪 1 MMR shot)

• Rubeola/ Measles Lab (not immune 🡪 2 MMR shots🡪one month wait in-between

• PPD Lab (positive 🡪 X-ray Letter) or QFT Lab (within current year)

RN Submission Requirements
